Receipt of Applications. The City will receive written and sealed Applications to furnish all labor, equipment, materials, tools, insurance, supervision, and all other items incidental thereto, and to perform all work necessary and specified in the prescribed manner and time to provide Non-Exclusive Temporary Debris Box and Roll-off Collection Service in the City Service Area in accordance with the terms and conditions set forth in the Agreement. Sealed Applications will be received until 4:00 p.m. PT, September 15, 2016, at the office listed below, and electronic copies will be received via email at the three email addresses listed below. Two (2) hard copies of each Application and supporting documentation must be submitted in a sealed envelope or container plainly labeled in the lower-left corner: "APPLICATION FOR NON- EXCLUSIVE TEMPORARY DEBRIS BOX AND ROLL-OFF COLLECTION SERVICES" along with the Application submittal date and time. Licensees must also include their company name and address on the outside of the envelope or container. Applicants also get information in the website xxxx://xxxx.xxxx.xxxxx.xx/ and write an email to the xxxx-xxxxxxxxxxx@xxxxx.xx. Applicants fill and submit the on-line application form uploading all the needed documents in the web portal xxxx://xxxx.xxxx.xxxxx.xx/submissions/index.php. The Co-ordinator receives all the application forms and checks the formal criteria for admission. All Institutions have access to the form and the uploaded documents through the web portal. Any administrative decision taken on the basis of the information provided by the applicant in the application form, but without a supporting document, will be conditionally accepted upon the presentation of the corresponding documentation. Such documentation will be requested in the acceptance letter, along with a deadline to provide it. Deadlines will be stated on a case-by-case basis.

Each application will be date and time stamped when received to ensure that only applications received by the application deadline are considered for funding, as stated on the FOA.
