Form of Application. The Contractor shall periodically submit to the Design Professional an Application for Payment on the form set forth in Section 7 (sometimes called a "Periodical Estimate") for each payment requested, and, if requested by the Owner or Design Professional, shall attach backup materials including, but not limited to, receipts or other vouchers, showing his payments for materials and labor, including payments previously made to Subcontractors.
Form of Application. Any individual may apply to take an examination by filing a prescribed form on which he/she states that he/she meets the announced requirements for such examination and that he/she understands that he/she will be eliminated at any stage in such examination if it develops that in fact he/she does not meet the announced requirements. This form must be filed at the office of the Commission on or before the closing date specified in the examination announcement, except that when a sufficient number of applications have not been received up to the closing date, the director may accept applications in the period between the closing date and the scheduled date of the examination. Otherwise applications filed after the closing date may be accepted only on approval of the Commission. When an examination has been announced as a continuous examination as provided in rule 1436, the closing date shall be indefinite and the period of applying shall continue until suspended or terminated by order of the Commission. This rule shall not apply where an examination is held entirely on an oral basis, or where other special circumstances make the procedure not feasible. In such case, each applicant shall file the official application form referred to in rule 1332 before the announced closing date. In promotional examinations, the preliminary forms shall be checked against personnel records, and any applicant who does not meet the announced requirements shall be disqualified in advance.

The Design-Builder shall periodically submit to the Program Manager an Application for Payment on the form set forth in Exhibit K for each payment requested, and, if requested by the Owner or Program Manager, shall attach backup materials including, but not limited to, receipts or other vouchers, showing his payments for materials and labor, including payments previously made to Subcontractors.
Form of Application. Prospective consumers must request service on the Cooperative’s standard application or service contract form. The application or contract for service shall be binding only after acceptance by a duly authorized representative of the Cooperative. The Cooperative requires a separate application or contract for each class of service at each separate location.
Form of Application. Application shall be concise and shall state:
1. Rationale or goals which the person intends to achieve.
2. Need of the school system or need of the individual to undertake the sabbatical.
3. Plan for achieving the goal or goals set forth.
4. Plans for implementing the findings and/or disseminating information upon return.
Form of Application. The Payment Application shall include a Cost Summary showing each GMP Cost Category and shall include a cost breakdown by CSI Category. CM/GC shall attach backup materials to each Payment Application to substantiate all Allowable Costs, which backup may include, but not be limited to receipts or other vouchers showing payments for materials and labor, including payments made to Trade Contractors.
Form of Application. Design-Builder shall, on or before the twenty-fifth (25th) day of each month, submit to Owner an itemized Application for Payment, completed in accordance with the Contract Documents and the approved Schedule of Values. Each application shall be supported by such substantiating data as Owner may require. When Design-Builder submits its monthly Application for Payment, it shall include, in addition to other requirements of the Contract Documents the following: (a) a completed Affidavit of Amounts Paid Minority and Women’s Business Enterprises (MBE/WBE) form and such other requirements as are identified in Article 22; (b) a completed Statement of Apprentice/Journeyman Participation form and such other requirements as set forth in Article 23; and (c) a waiver and release of claims and mechanic’s liens. Payments will not be considered due and payable by Owner unless these forms are properly completed and timely received by Owner.
Form of Application. The application for assistance will be in the form of a letter to the FmHA or its successor agency under Public Law 103–354 County Supervisor having jurisdiction over the area of the proposed site to be optioned. The letter will be signed by the applicant or its authorized represent- ative and contain, as a minimum, the fol- lowing information:
(1) A copy of the proposed option that shows a legal description of the land, option price, purchase price, and terms of the op- tion. If more than one site is to be pur- chased, a schedule of the proposed options should be included.
(2) Information to verify that a regular RHS loan cannot be processed in time to se- cure the option.
(3) Proposed method repayment of the SO loan.
(4) Resolution from the applicant’s gov- erning body authorizing the application for an SO loan from FmHA or its successor agency under Public Law 103–354.
