If an employee has been provided safety glasses by the Citynot suitable, the employee shall be permitted to retain possession of promptly notify the glasses after separation from the City without reimbursing the City for any costs associated with the glassesHuman Resources Department.

Section 520.1.2.5 Replacement of lost glasses will be at employee expense, unless the employee's supervisor otherwise recommends, based on evidence that the loss occurred during the performance of work for reasons other than employee carelessness or negligence. In the event a probationary employee has been issued Non-glare safety glasses and terminates his employment with the City for any reason during the probationary period, he shall be required provided to reimburse VDT users if the City for any expenses incurred Safety Committee so recommends.
20.1.2.6 Employees not participating in the Safety Glasses Program will have the option of receiving an equal contribution towards the purchase of frames and lenses through a designated vendor as authorized by the Director of the Human Resources Department. Employees may purchase non-safety glassesframes and lenses through a vendor of their choice, however the City’s contribution for reimbursement will be equal to that of the designated vendor used for the Safety Glasses Program. Non-safety Glasses may only be replaced upon furnishing proof of a change in prescription; no more than one pair per year to be replaced on this basis. No provisions for the repair or replacement of non-safety glasses for any other reason, including damage, wear and tear, and loss. The City retains the right to make determinations regarding qualifying expenses.

10.3.3 Employees who wear prescription glasses will be provided with appropriate eye protection that can be worn over the prescription lenses without disturbing the proper position, fit, or usefulness of the prescription lenses. 10.3.4 Employees who wear prescription glasses, whose proper position, fit, or usefulness is impeded by eye protection worn over prescription lenses as prescribed. 15.06 The Employer agrees to exceed $75.00maintain at the work place the first aid equipment as required by Law.
15.07 When a wage earner is victim of an accident at work he shall have no reduction in pay for the day of the accident. This excludes If he is sent home, to the hospital or to a physician, the Employer shall pay the additional cost of transportation.
15.08 When a wage earner who has suffered an accident at work must, following his return to work, go to the eye examination which will be the responsibility of the employee. The effected employees will be allowed one (1) replacement of safety glasses every two (2) years. In the event the safety glasses become lost, unserviceablephysician's office, or broken on the jobhospital for examination or treatment, the employee must present a written request for replacement to the Department Head and Human Resources Director. 15.09 When a wage earner is to receive weekly disability payments under Workmen's Compensation (CSST), the Employer will advance such payments on a weekly basis, if requested by the wage earner, until such benefits are regularly paid to him.
15.10 The replacement of lost glasses or glasses that are broken off the job will be at the discretion duties of the Department Head safety representative are:
1) to inspect the work premises;
2) to receive copy of all notices of accident and Human Resources Director. If an employee has been provided safety glasses to investigate the possible causes of the accident;
3) to identify the situations which may be a source of danger to the workers;
4) to make the recommendations he deems appropriate to the Health and Safety Committee or, failing such Committee, to the workers or to their certified association and to the Employer;
5) to assist the workers in the exercise of their recognized rights pursuant to the present law and regulations;
6) to accompany the inspector during his inspections;
7) to intervene in cases where the worker exercises his right to refusal;
8) to file a complaint with the Chief Regional inspector;
9) to participate in the identification and the evaluation of the characteristics concerning the work premises and the work performed by the Cityworkers as well as the identification of any contaminants and dangerous materials which are present on the work premises for the purpose of article 52 of the Act respecting Occupational Health and Safety.
15.11 The safety representative may leave his work, without loss of salary, for such time as is necessary, in order to fulfil his duties pursuant to article 15.10 unless his duties are exercised abusively.
15.12 The safety representative may not be subjected to a layoff, dismissal, suspension, transfer or disciplinary or discriminatory measure on account of the performance of his duties unless his duties are exercised abusively.
15.13 The members of the Health and Safety Committee have preferential seniority in case of layoff. Furthermore, the employee shall be permitted to retain possession of the glasses after separation from the City without reimbursing the City for any costs associated with the glasses.
Section 3. An employee who Union Co-President is required to wear prescription safety glasses must present a written request to his department head or designated representative.
Section 4. The employee must obtain a current prescription and the employee is authorized the use of sick leave not to exceed two (2) hours to accomplish this examination. Safety Glasses. Section 1A. Those employees designated by eBART as required to wear eye protection and who presently wear prescription glasses will be furnished prescription safety glasses.
B. Those employees who presently wear prescription glasses will be required to take an eye examination and utilize whatever medical coverage they have that includes this provision. If the employee's medical coverage does not include eye examinations, the District will provide; however, this does not include “first-time” eye examination.
C. Safety prescription glasses provided under this program shall not be altered without District approval.
D. Prescription or Plano safety glasses shall be worn in those locations designated by the Chief Safety Officer or designee, who will set reasonable requirements based on work actually performed by each classification, and the applicable law and regulations.
