Safety Glasses. Section 1. The City shall supply prescription safety glasses with plastic lenses to employees who are required to wear safety glasses and who are members of the classifications contained in Appendix C to this contract. Safety glasses which are authorized must be industrial grade safety glasses which meet or exceed the requirements of ANSI Specification Z87. 1. This excludes the cost of the eye examination which will be the responsibility of the employee. The effected employees will be allowed one (1) replacement of safety glasses every two (2) years. In the event the safety glasses become lost, unserviceable, or broken on the job, the employee must present a written request for replacement to the Department Head and Human Resources Director. If the employee breaks his safety glasses while on the job, the Department shall replace the glasses at no cost to the employee. The replacement of lost glasses or glasses that are broken off the job will be at the discretion of the Department Head and Human Resources Director. If an employee has been provided safety glasses by the City, the employee shall be permitted to retain possession of the glasses after separation from the City without reimbursing the City for any costs associated with the glasses. In the event a probationary employee has been issued safety glasses and terminates his employment with the City for any reason during the probationary period, he shall be required to reimburse the City for any expenses incurred in the purchase of safety glasses.

Employees may purchase non-safety glassesframes and lenses through a vendor of their choice, however the City’s contribution for reimbursement will be equal to that of the designated vendor used for the Safety Glasses Program. Non-safety Glasses may only be replaced upon furnishing proof of a change in prescription; no more than one pair per year to be replaced on this basis. No provisions for the repair or replacement of non-safety glasses for any other reason, including damage, wear and tear, and loss. The City retains the right to make determinations regarding qualifying expenses.

(California Code of Regulations, Title 8, section 3382) 10.3.2 Employees who primarily work outdoors do not wear prescription glasses will be provided with appropriate eye protection as approved by the Santa Xxxxxxx County XXXX safety officer. 10.3.3 Employees who wear prescription glasses will be provided with appropriate eye protection that can be worn over the prescription lenses without disturbing the proper position, fit, or usefulness of the prescription lenses. Appropriate eye protection will be the type approved by Santa Xxxxxxx County XXXX safety officer. 10.3.4 Employees who wear prescription glasses, whose proper position, fit, or usefulness is impeded by eye protection worn over prescription lenses as prescribed. In the event that additional classes are identified as needing either provided in 10.3.3 will be provided with prescription safety glasses or protective eyewear, such classes may be added to from a vendor selected by the classification list in Appendix C upon approval of PAGE district and approved by the CitySanta Xxxxxxx County XXXX safety officer. Section 2. 10.3.5 The City agrees to pay the full cost of required employee who must wear prescription safety glasses, with frames not to exceed $75.00. This excludes the cost of the eye examination which glasses will be responsible to provide the responsibility of the employee. In the event the safety glasses become lost, unserviceable, or broken on the job, the employee must present a written request for replacement to the Department Head and Human Resources Director. The Employer also agrees to provide the wage earner with a new pair of prescription safety glasses when the wage earner will have worn such glasses for two years and where their replacement is required due to normal wear. In any other circumstances, such classes may the replacement cost of prescription safety glasses will be added borne by the Employer up to a maximum of 50% of their cost to the classification list in Appendix C upon approval of PAGE and Employer. The City agrees to pay wage earner chooses himself the full cost of required prescription safety glasses, either with frames or without prescription, the Employer's contribution shall in no circumstances exceed the latter's cost. The Employer will not pay for any additional cost for progressive lens glasses. 15.06 The Employer agrees to exceed $75.00maintain at the work place the first aid equipment as required by Law. 15.07 When a wage earner is victim of an accident at work he shall have no reduction in pay for the day of the accident. This excludes If he is sent home, to the hospital or to a physician, the Employer shall pay the additional cost of transportation. 15.08 When a wage earner who has suffered an accident at work must, following his return to work, go to the eye examination which will be the responsibility of the employee. Furthermore, the employee shall be permitted to retain possession of the glasses after separation from the City without reimbursing the City for any costs associated with the glasses. Section 3.
