Schedule of Supports. 1.1. Provided that NDIS funds are available for NDSP to access on the NDIA portal relevant to the Client, NDSP agrees to provide the supports to the Client as set out in Annexure B below.
1.2. The prices set out in Annexure B are GST inclusive (if GST applies) and include the fees for providing the stated supports. The fees will be automatically updated from time-to-time in line with changes to the NDIA Price Limits and/or other documents or publications issued by the NDIA from time-to-time.

Schedule of Supports. The Provider agrees to provide to the Participant and the Participant agrees to receive the Supports set out in the Supports Schedule in the Registration Form. The Provider agrees, to provide the Participant support, through their programs, in: • Assistance with Social, Community & Civic Participation - Category 4 • Increased Social & Community Participation - Category 9 • Improved Daily Living - Category 15 • Improved Relationships - Category 11
Schedule of Supports. Participate Australia agrees to provide the Participant the types of supports outlined in the Participant’s NDIS Plan and agreed upon for each support booking. Prices are based on the NDIS Price Guide that is current at the time supports are delivered. The supports and their prices are set out in the attached Quotation (Appendix III). Prices and any subsequent changes are based on the NDIS guidelines. All prices are GST inclusive (if applicable) and include the cost of providing the supports.
Schedule of Supports. Provided the funds are available for GA Business Services T/As Allround Plan Management to access on the National Disability Insurance Agency (NDIA) portal relevant to the participant /nominated representative, GA Business Services T/As Allround Plan Management agrees to provide supports relating to the Clients Plan Management – Financial Administration, set out in the attached Schedule of Supports. The prices for those supports are set out in the Schedule of Supports and are GST inclusive (if applicable) and include the cost of providing the supports, Additional expenses (i.e., things that are not included as part of a participant’s NDIS supports) are the responsibility of the participant /nominated representative and are not included int the cost of the supports.
Schedule of Supports. Rise agrees to provide the Client with a description and duration of each of the supports provided and this will be broken-down between funded and unfunded supports. The supports and their prices are set out in Appendix 1: Schedule of Supports and (if applicable) may include the cost of providing the supports. NDIS funded items are listed and charged, excluding GST. Privately funded supports may incur GST. Additional expenses or ‘unfunded supports’ (i.e. things that are not included as part of a Client’s NDIS funded supports) are the responsibility of the Client / Client’s legal representative and are listed in the Schedule of Supports.
Schedule of Supports. The provider agrees to provide music therapy sessions at TIME: on DATE: & FREQUENCY: , with the usual location being LOCATION: . Variations by either party to the time and/or day and/or location, due to sickness or school/public holidays etc, will be negotiated on a case by case basis, to suit both parties. The need for the participant to be accompanied to these sessions by a support worker is to be discussed. The supports and their prices are set out in the attached Schedule of Supports. Additional expenses (i.e. things that are not included as part of a Participant’s NDIS supports) are the responsibility of the participant / participant’s representative.
Schedule of Supports. The provider agrees to provide the participant Supported Independent Living Supports for the period of the NDIS plan. The supports and their prices are set out in the attached Schedule of Supports. All prices are GST inclusive (if applicable) and include the cost of providing the supports.
