Schedule of Supports. 1.1. Provided that NDIS funds are available for NDSP to access on the NDIA portal relevant to the Client, NDSP agrees to provide the supports to the Client as set out in Annexure B below.

Schedule of Supports. Participate Australia agrees to provide the Participant the types of supports outlined in the Participant’s NDIS Plan and agreed upon for each support booking. Prices are based on the NDIS Price Guide that is current at the time supports are delivered. The supports and their prices are set out in the attached Quotation (Appendix III). Prices and any subsequent changes are based on the NDIS guidelines. All prices are GST inclusive (if applicable) and include the cost of providing the supports.
Schedule of Supports. This Provider agrees to provide the Participant supports that aim to support their independence and development for the duration of this agreement or until directed by the Participant and/or their carer. These services can include: • Occupational Therapy assessment: parent interview + participant assessment; • Occupational Therapy intervention: clinic, preschool/daycare/school, home based or telehealth; • Occupational Therapy groups – clinic based; • Any reports or letters as required by the NDIS or requested by the Participant’s representative; • Attendance at any team meetings / case conferences; • Assessment and trial of equipment; • Clinically relevant communications (including, emails and/or phone calls); • Travel charges for outreach services as per the current NDIS Price Guide; • Cancellation charges for late notice or no show appointments as per the current NDIS price guide; • Treatment planning and family inclusion: As part of our obligations as a registered NDIS Providers we must develop and review treatment plans which are to be provided to you. The prices for those supports are set out in the Schedule of Supports and are GST inclusive (if applicable) and include the cost of providing the supports, Additional expenses (i.e., things that are not included as part of a participant’s NDIS supports) are the responsibility of the participant /nominated representative and are not included int the cost of the supports.

Feedback, complaints and disputes If the Participant wishes to give the Provider feedback or make a complaint, the Participant or Representative can speak to Xxxxx Widloecher or Xxxxxx Xxxxx on 0000 000 000 or make contact via email (Xxxxx@XxxxxxxxxxxXX.xxx.xx) or in writing (PO Xxx 000, Xxxxx Xxxxxx XXX 2519). If the Participant is not satisfied with the response or does not wish to talk to these contacts, the Participant can contact the National Disability Insurance Agency by calling 1800 800 110, visiting one of their offices in person, or visiting xxxx.xxx.xx for further information. Changes to this Service Agreement If changes to the Supports or their delivery are required, the Parties agree to discuss and review this Service Agreement. The Parties agree that any changes to this Service Agreement will be in writing, signed, and dated by the parties. Ending this Service Agreement Should either Party wish to end this Service Agreement they must give 2 weeks Notice. If either party seriously breaches this Service Agreement the requirement of Notice will be waived.
