Schedule of Supports. Participate Australia agrees to provide the Participant the types of supports outlined in the Participant’s NDIS Plan and agreed upon for each support booking. Supports and costs will be determined and agreed upon each plan and quote. The supports and their prices are set out in the attached Quotation (Appendix II). Prices and any subsequent changes are based on the NDIS guidelines. All prices are GST inclusive (if applicable) and include the cost of providing the supports.

Schedule of Supports. Provided the funds are available for GA Business Services T/As Allround Plan Management to access on the National Disability Insurance Agency (NDIA) portal relevant to the participant /nominated representative, GA Business Services T/As Allround Plan Management agrees to provide supports relating to the Clients Plan Management – Financial Administration, set out in the attached Schedule of Supports. The prices for those supports are set out in the Schedule of Supports and are GST inclusive (if applicable) and include the cost of providing the supports, Additional expenses (i.e., things that are not included as part of a participant’s NDIS supports) are the responsibility of the participant /nominated representative and are not included int the cost of the supports.

As these policies may change from time to time, current policy is detailed on Service Schedules. Please refer to the current Service Schedule for information regarding current Travel Fee Policy. Feedback, complaints and disputes If the Participant wishes to give the Provider feedback or make a complaint, the Participant or Representative can speak to Xxxxx Widloecher or Xxxxxx Xxxxx on 0000 000 000 or make contact via email (Xxxxx@XxxxxxxxxxxXX.xxx.xx) or in writing (PO Xxx 000, Xxxxx Xxxxxx XXX 2519). If the Participant is not satisfied with the response or does not wish to talk to these contacts, the Participant can contact the National Disability Insurance Agency by calling 1800 800 110, visiting one of their offices in person, or visiting xxxx.xxx.xx for further information. Changes to this Service Agreement If changes to the Supports or their delivery are required, the Parties agree to discuss and review this Service Agreement. The Parties agree that any changes to this Service Agreement will be in writing, signed, and dated by the parties. Ending this Service Agreement Should either Party wish to end this Service Agreement they must give 2 weeks Notice. If either party seriously breaches this Service Agreement the requirement of Notice will be waived.
