Second Year Model Testing Pre-Approval Requirements. The Recipient had to submit an Operational Plan by December 1, 2017. The Recipient agrees to address deficiencies in their Operational Plan as identified by CMMI, provide clarification on specified elements of their Operational Plan, and provide evidence to demonstrate their readiness for the second year of the Model Testing Phase. The submitted evidence should describe how broad-based accountability for outcomes, including total cost of care for Medicare, Medicaid, and CHIP beneficiaries, is created. In addition, submitted evidence for new payment and service delivery models must describe a pathway with specific milestones to move the care in the state for the preponderance of the state’s total population from models that reward service volume to clinical and financial models that reward better health, better care, and lower cost through improvement, and consider levers and strategies that can be applied to influence the structure and performance of the state’s entire health care system, as stated in the FOA. The Recipient must cooperate with CMS and its contractors to ensure that the submitted evidence demonstrates their readiness for the second year of the Model Testing Phase. Specifically, during the second year of the Model Testing Phase the Recipient agrees to submit evidence outlining processes and milestones including, but not limited to, the following areas:

Second Year Model Testing Pre-Approval Requirements. Recipient had to submit a letter of attestation, obligating Recipient to be accountable for: All proposed SIM activities included in CT’s original SIM proposal, as amended (which constitutes the basis for granting the state a SIM R2 award); all components of the state’s SIM R2 Operational Plans from 2015-2017 (continuing to implement and maintain activities outlined in those AY1 and AY2 plans), as amended; completion of all carryover milestones from AY1 and AY2; maintenance of AY1 and AY2 data and activities; and completion of AY3 and AY4 milestones/activities.   • Commercial Operation Date Testing and Modifications Prior to the Commercial Operation Date, the Connecting Transmission Owner shall test the Connecting Transmission Owner’s Attachment Facilities and System Upgrade Facilities and System Deliverability Upgrades and Developer shall test the Large Generating Facility and the Developer Attachment Facilities to ensure their safe and reliable operation. Similar testing may be required after initial operation. Developer and Connecting Transmission Owner shall each make any modifications to its facilities that are found to be necessary as a result of such testing. Developer shall bear the cost of all such testing and modifications. Developer shall generate test energy at the Large Generating Facility only if it has arranged for the injection of such test energy in accordance with NYISO procedures.

  • Post-Commercial Operation Date Testing and Modifications Each Party shall at its own expense perform routine inspection and testing of its facilities and equipment in accordance with Good Utility Practice as may be necessary to ensure the continued interconnection of the Large Generating Facility with the Participating TO’s Transmission System in a safe and reliable manner. Each Party shall have the right, upon advance written notice, to require reasonable additional testing of the other Party’s facilities, at the requesting Party’s expense, as may be in accordance with Good Utility Practice.

  • Minimum System Requirements The following summarizes the minimum office system requirements for all Contractors and Architect/Engineer to possess in order to participate.   • Minimum Vendor License Requirements Vendor shall maintain, in current status, all federal, state, and local licenses, bonds and permits required for the operation of the business conducted by Vendor. Vendor shall remain fully informed of and in compliance with all ordinances and regulations pertaining to the lawful provision of goods or services under the TIPS Agreement. TIPS and TIPS Members reserve the right to stop work and/or cancel a TIPS Sale or terminate this or any TIPS Sale Supplemental Agreement involving Vendor if Vendor’s license(s) required to perform under this Agreement or under the specific TIPS Sale have expired, lapsed, are suspended or terminated subject to a 30‐day cure period unless prohibited by applicable statue or regulation.

Cleanup: When performing work on site at a TIPS Member’s property, Vendor shall clean up and remove all debris and rubbish resulting from their work as required or directed by the TIPS Member or as agreed by the parties. Upon completion of work, the premises shall be left in good repair and an orderly, neat, clean and unobstructed condition. Preparation: Vendor shall not begin a project for which a TIPS Member has not prepared the site, unless Vendor does the preparation work at no cost, or until TIPS Member includes the cost of site preparation in the TIPS Sale Site preparation includes, but is not limited to: moving furniture, installing wiring for networks or power, and similar pre‐installation requirements. Registered Sex Offender Restrictions: For work to be performed at schools, Vendor agrees that no employee of Vendor or a subcontractor who has been adjudicated to be a registered sex offender will perform work at any time when students are, or reasonably expected to be, present unless otherwise agreed by the TIPS Member. Vendor agrees that a violation of this condition shall be considered a material breach and may result in the cancellation of the TIPS Sale at the TIPS Member’s discretion. Vendor must identify any additional costs associated with compliance of this term. If no costs are specified, compliance with this term will be provided at no additional charge. Safety Measures: Vendor shall take all reasonable precautions for the safety of employees on the worksite, and shall erect and properly maintain all necessary safeguards for protection of workers and the public.   • Ongoing Trunk Forecast Requirements Where the Parties have already established interconnection in a LATA, Onvoy shall provide a new or revised traffic forecast that complies with the Frontier Interconnection Trunking Forecast Guide when Xxxxx develops plans or becomes aware of information that will materially affect the Parties’ interconnection in that LATA. Instances that require a new or revised forecast include, but are not limited to: (a) Onvoy plans to deploy a new switch; (b) Onvoy plans to implement a new POI or network architecture; (c) Onvoy plans to rearrange its network; (d) Onvoy plans to convert a One-Way Interconnection Trunk group to a Two-Way Interconnection Trunk group; (e) Onvoy plans to convert a Two-Way Interconnection Trunk group to a One-Way Interconnection Trunk group; or (f) Onvoy expects a significant change in interconnection traffic volume. In addition, upon request by either Party, the Parties shall meet to: (i) review traffic and usage data on End Office and Tandem Interconnection Trunk groups and (ii) determine whether the Parties should establish new Interconnection Trunk groups, augment existing Interconnection Trunk groups, or disconnect existing Interconnection Trunks.

  • Trunk Forecasting Requirements 14.2.1 Initial trunk forecast requirements. At least ninety (90) days before initiating interconnection in a LATA, Alltel shall provide Verizon a two (2)-year traffic forecast that complies with the Verizon Interconnection Trunking Forecast Guide, as revised from time to time. This initial traffic forecast will provide the amount of traffic to be delivered to and from Verizon over each of the Interconnection Trunk groups in the LATA over the next eight (8) quarters.
