Service Coordination Team. Led by a Service Coordinator and ensures the integration of the Enrollee’s medical, behavioral health, substance use, LTSS, and social needs. The Service Coordination Team includes the Enrollee’s PCP, regardless of whether the PCP is in the STAR+PLUS MMP’s network during the transition period. The Service Coordination Team coordinates all Covered Services and any applicable Non-Capitated Services. The Service Coordination Team is person-centered, built on the Enrollee’s specific preferences and needs, and delivers services with transparency, individualization, respect, linguistic and cultural competence, and dignity.

Every Enrollee shall have access to and input in the development of their own Service Coordination Team. If the STAR+PLUS MMP is unable to reach an Enrollee, or if the Enrollee is unwilling to participate in the creation of a Service Coordination Team, the STAR+PLUS MMP is not required to develop a Service Coordination Team but must continue to provide a Service Coordinator for the Enrollee as described in Section 2.5.3.2.

Within 14 days of the effective date of this Consent Agreement, DTSC and Respondent shall each designate a Project Coordinator and shall notify each other in writing of the Project Coordinator selected.
