SITE OF SERVICE for the purposes of this agreement, includes these three types of pharmacies: • retail pharmacies; • specialty pharmacies; and • mail order pharmacy.
SITE OF SERVICE. This fee schedule applies no variation in pricing based on where the service is performed. United routinely updates its fee schedule in an effort to stay abreast of current coding practices widely used in the health care industry; in response to price changes for immunizations and injectable medications; and to remain in compliance with the intent of the contractual agreement. Routine maintenance occurs when United mechanically incorporates revised information created by a third party that is the source for a portion of the fee schedule. United will not generally attempt to communicate routine maintenance of this nature and will generally implement updates to be effective within 90 days from the date of final publication from one of our primary or alternate fee sources. Providers may expect the following types of fee updates to their fee schedules:

SITE OF SERVICE. Firebird Earth Medicine shall provide the selected retreat program services at the following site: 0000 Xxxxxxx Xxxx, Xxxxxxxx, Colorado 81415. The site includes a 40- acre fenced property, residences, art studio, dance studio, out buildings, barn, and arena.
SITE OF SERVICE. Effective January 1, 2019; Site of Service (SOS) network/plan shall be applied to all health insurance plans other than the high deductible plan (aka HSA plan) as set forth in the attached Exhibit B.
SITE OF SERVICE. Gateways to Transformation, LLC shall provide the selected program services at the following site: 0000 Xxxxxxx Xxxx, Xxxxxxxx, Colorado 81415. The site includes a 40- acre fenced property, residences, treatment facilities, out buildings, barn, and arena. If payment is delinquent for 30 days or more there will be a finance charge of twelve Percent (12%) per annum added to the out-standing balance until the balance is paid in full.
