If a completed Contractor Performance Assessment Reporting System (CPARS) evaluation is available, it shall be submitted with the proposal for each project included in Factor 1, Corporate Experience. If there is not a completed CPARS evaluation then submit Past Performance Questionnaire (PPQ Attachment C) for each project included in Factor 1. The Offeror should provide completed PPQs in the proposal. Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs.
