Solicitation Submittal Requirements. The offeror shall submit the following information: The Days Away from Work, Restricted Duty, or Job Transfer (DART) Rate; and Total Case Rate (TCR) for the specified five (5) Calendar Years (CY), as well as a safety narrative, as described further below. For a partnership or joint venture, the offeror shall submit separate DART rates and TCR for the specified five (5) CY for each contractor who is part of the partnership/joint venture; however, only one safety narrative is required. Any fatalities experienced within this 5-year timeframe must be explained in detail, to include root cause and corrective actions. NOTE: DART and TCR shall not be submitted for subcontractors

Solicitation Submittal Requirements. FOR EACH PROJECT SUBMITTED IN RESPONSE TO FACTOR 1, CORPORATE EXPERIENCE, IF A COMPLETED CPARS EVALUATION IS AVAILABLE, IT SHALL BE SUBMITTED WITH THE PROPOSAL. If there is not a completed CPARS evaluation, the Past Performance Questionnaire (PPQ) included in the solicitation is provided for the offeror or its team members to submit to the client for each project the offeror includes in its proposal for Factor 1, Corporate Experience. AN OFFEROR SHALL NOT SUBMIT A PPQ WHEN A COMPLETED CPARS IS AVAILABLE. If a CPARS evaluation is not available, ensure correct phone numbers and email addresses are provided for the client point of contact. Completed PPQs should be submitted with your proposal. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror should complete and submit with the proposal the first page of the PPQ (Attachment JM-2), which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires. If the client requests, questionnaires may be submitted directly to the Government's point of contact, Xxxx Xxxxxx or Xxxx Xxxxxxx via email at xxxx.xxxxxx@xxxx.xxx or xxxx.xxxxxxx@xxxx.xxx prior to proposal closing date. Offerors shall not incorporate by reference into their proposal PPQs or CPARS previously submitted for other RFPs. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation. In addition to the above, the Government may review any other sources of information for evaluating past performance. Other sources may include, but are not limited to, past performance information retrieved through the Contractor Performance Assessment Reporting System (CPARS) using all CAGE/Entity Identifier numbers of team members (partnership, joint venture, teaming arrangement, or parent company/subsidiary/affiliate) identified in the offeror’s proposal, inquiries of owner representative(s), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), and any other known sources not provided by the offeror. Offerors may submit past performance infromation on up to five projects/contracts that demonstrate the offeror’s ability to successfully deliver products and services that are similar to the work described in the performance work statement. For each project/contract, there are two submittal requirements: (1) past performance evaluation forms; and (2) a narrative statement.

If the offeror is a Joint Venture (JV), recent/relevant experience should be submitted for projects completed by the JV entity. If the JV does not have such experience, recent/relevant experience shall be submitted for each JV partner (except for offerors covered by Mentor-Protégé Agreements approved by the Small Business Administration). JV offerors are still limited to a maximum total of five (5) recent and relevant projects. If a project was performed by a JV and not all members from the prior JV are proposing on this solicitation, the offeror shall clearly demonstrate what portion of the work on the submitted project was performed by the JV partner proposing on this solicitation. Solicitation Submittal Requirements. The Project Data Sheet (found in Section J of the solicitation) is MANDATORY and SHALL be used to submit project information. Except as specifically requested, the Government will not consider information submitted in addition to this form. Individual blocks on this form may be expanded; however, total length for each Project Data Sheet shall not exceed two (2) pages. In the event the Project Data Sheet exceeds two (2) pages, Offerors will receive evaluation of the first two (2) pages and the remainder will not be evaluated. Offeror shall submit a minimum of two (2) and a maximum of five (5) service projects/work performed by the Offeror that best demonstrates Offeror experience on recent, relevant projects that are similar in size, scope and complexity to the Request for Proposal (RFP). For purposes of this evaluation, a relevant project is further defined as SIMILAR CUSTODIAL SERVICES performed under a single task order or contract. If the CUSTODIAL Services were performed as a part of a multiple award and IDIQ type contract, the contract as a whole shall not be submitted as a project, rather Offerors shall submit the work performed under a task order as a project and provide the specific services, tasks and value that is considered to be same or similar to the scope, size, and complexity to the RFP. Service projects submitted for the Offeror shall be completed within the past five (5) years of the date of issuance of this RFP. Any projects submitted in excess of the five (5) projects will not be considered. If the Offeror is a Joint Venture (JV), relevant project experience should be submitted for projects completed by the Joint Venture entity. If the Joint Venture does not have shared experience, projects may be submitted for the Joint Venture members. Offerors who fail to submit experience for all Joint Venture members will be found unacceptable. Offerors are still limited to a maximum of five (5) service projects/work combined. The Offeror shall submit a signed copy of the Joint Venture Agreement indicating the proposed participation of each Joint Venture member. Failure to submit the required Joint Venture Agreement will be found unacceptable.
