Target Population and Enrollment. Grantee will target a coverage-eligible population that includes those attributed to primary care practices in the Health Services Area and individuals identified as uninsured or underinsured at the point of care. Navigators or Community Health Teams may work with health systems when available to query their panel of patients to identify those who are coverage-eligible, including working with the primary care practices, addictions treatment centers, hospitals, or emergency departments.

The target population of Beehive Science & Technology Academy will be represented by various school districts.
