The Improvement Plan shall. i) Define the improvement goals directly related to the Performance Standard(s) and/or student learning outcomes that must be improved; ii) Describe the activities and work products the Educator must complete as a means of improving performance; iii) Describe the assistance that the district will make available to the Educator; iv) Articulate the measurable outcomes that will be accepted as evidence of improvement; v) Detail the timeline for completion of each component of the Plan, including at a minimum a mid-cycle Formative assessment report of the relevant standard(s) and indicator(s); vi) Identify the individuals assigned to assist the Educator which must include minimally the Primary Evaluator; vii) Include the signatures of the Educator and Primary Evaluator.
