Time Limits and Substantiation. Employees must submit a claim within thirty (30) days of occurrence. Any claim for repair or replacement shall be substantiated with evidence clearly indicating that the damage was caused by circumstances beyond the control of the employee. Receipts for payment of repair or replacement costs will be required.
