Use of Evidence Log. Any Evaluator who collects evidence shall maintain an Evidence Log for each Educator under his/her supervision using the Evaluator Record of Evidence Form in Appendix C-1. The Log shall be used to document and preserve evidence that may be relied upon to determine ratings against standards and/or to assess progress toward goals. A Log entry may include attachments. Any time an Evaluator makes an entry into a Log, the Educator will receive a copy of the Log entry and any relevant attachments. Evidence Collection and Recordkeeping Protocols Evaluators shall collect and record evidence from classroom observations and other sources of evidence allowed under state regulations according to the following protocols: Classroom observations Classroom observations may be announced or unannounced and of any duration.
