Your Physician will provide you with a personalized wellness plan, to be updated at your annual visits, focusing on appropriate screening and early detection of medical problems leading to early diagnosis and treatment. The preparation of the wellness plan is at no additional cost to you; but you will be financially responsible for those services outlined in your wellness plan which are not covered by your health insurance plan.
