Work-Related Illness or Injury Absence. Should medical attention become necessary, the employee should take the Bureau of Workers' Compensation (BWC) forms provided in the Franklin County Injury Packet to their treating medical provider. If additional BWC forms are needed they can be obtained through the Agency’s Payroll Clerk or the online employee portal. Any absence from work as a result of a work-related injury must be documented through a physician’s excuse from work letter, or a completed MEDCO-14 form, which can be found in the County’s Injury Packet, or with the Agency’s Payroll Clerk. All completed accident reports or BWC forms should be submitted to the employer within 48 hours.

If performance of this Agreement involves construction, or the transport of persons or hazardous materials, limits of coverage shall not be less than $1,000,000 combined single limit.
