By voluntarily entering into this Agreement and remitting the Annual Retainer Fee (as set forth below), you may participate in the PPC Atlanta’s Personalized Primary Care Program (the “Program”) for a period of twelve (12) months beginning on the Effective Date.
Subject to the provisions of Article VIII of the Plan, the maximum number of shares of Common Stock which may be issued under the Plan in payment of a Director's Annual Retainer Fee or upon exercise of Stock Options shall not exceed 200,000 shares (the "Share Limit").
