Community Developmental Disability Program definition

Community Developmental Disability Program or "CDDP" means an entity that is responsible for planning and delivery of services for persons with mental retardation or other developmental disabilities in a specific geographic area of the state under a contract with the Department or a local mental health authority.

Community Developmental Disability Program or “CDDP” has the meaning as set forth in OAR 000-000-0000.

• Services and supports may range from information and referral, to case management, or hourly services, to assist an individual to live in their own home, access employment or other day services.ODDS Service Recipient (ODDS Client): An individual with I/DD found eligible for services may access ODDS services through Community Developmental Disability Program (CDDP), Support Services Brokerage, or other services.

• All Community Developmental Disability Program (CDDP) contractors providing developmental disability services under a contract with the Department of Human Services are required to meet the basic management, programmatic, health, safety and human rights regulations in the management of the community service system for individuals with developmental disabilities.

• All financial and timekeeping records and all other documentation pertaining to services rendered under this Agreement shall be made available to DHS, OHA, the Recipient’s case managing Community Developmental Disability Program (CDDP), Recipient’s brokerage, Children’s Intensive In-Home Services (CIIS), Oregon Department of Justice Medicaid Fraud Unit, the Oregon Secretary of State’s Office and the federal government, and their duly authorized representatives to examine, audit and make copies upon demand.

• All financial and timekeeping records and all other documentation pertaining to services rendered under this Agreement shall be made available to DHS, the Recipient’s case managing Community Developmental Disability Program (CDDP), Recipient’s Brokerage, Oregon Department of Justice Medicaid Fraud Unit, the Oregon Secretary of State’s Office and the federal government, and their duly authorized representatives to examine, audit and make copies.

• In the case of a serious illness, injury or death of an individual, the Brokerage or Provider Organization must immediately notify:(A) The individual's legal guardian or conservator, parent, next of kin, designated contact person or other significant person;(B) The Community Developmental Disability Program; and(C) In the case of the Provider Organization, the individual’s Support Services Brokerage.Stat.

• Be placed with the agreement of the Community Developmental Disability Program if the individual is either private pay or not developmentally disabled.

• The In-Home Support Plan is developed by the Community Developmental Disability Program and individual, the individual's legal representative, (if applicable) or family to articulate decisions and agreements made during a person-centered process of planning and information gathering.

• Documentation from the Oregon Department of Human Services (DHS) or a DHS-designated Community Developmental Disability Program that the individual has an existing disability.

• These rules prescribe general administrative standards for operation of a Community Developmental Disability Program (CDDP) operated by or on behalf of a Local Mental Health Authority (LMHA).
