Community Longitudinal Family Physician Services definition

Community Longitudinal Family Physician Services or “Community LFP Services” means the types of services typically provided by a full-service community family physician who provides longitudinal heath care services (including medically required services to beneficiaries) and those services associated with a Patient Medical Home, including the services described in Section 12 [Included Services] but excluding those services described in Section 13 [Excluded Services].
Community Longitudinal Family Physician Services means the types of services typically provided by a longitudinal family physician who provides longitudinal, relationship- based, comprehensive, family medicine care in a community based setting health care services (including medically required services to beneficiaries), and those services associated with aligned with the attributes of a Patient Medical Home.

  • Primary care physician means a physician qualified to be an attending physician according to ORS 656.005(12)(b)(A) and who is a general practitioner, family practitioner, or internal medicine practitioner.

  • Naturopathic physician means a person licensed to practice naturopathic medicine by the Oregon Board of Naturopathic Medicine.

  • Assisted living services means services provided by a

  • Medical physicist means a person trained in evaluating the performance of mammography equipment and facility quality assurance programs and who meets the qualifications for a medical physicist set forth in 41.6(3)“c.”

  • Home health aide services means the personal care and maintenance activities provided to individuals for the purpose of promoting normal standards of health and hygiene.

  • Specialist Physician means a licensed physician who qualifies as an attending physician and who examines a patient at the request of the attending physician or authorized nurse practitioner to aid in evaluation of disability, diagnosis, or provide temporary specialized treatment. A specialist physician may provide specialized treatment for the compensable injury or illness and give advice or an opinion regarding the treatment being rendered, or considered, for a patient’s compensable injury.

  • Family caregiver means a relative by blood, marriage, or Adoption who lives with or is the primary Caregiver of the terminally ill Member.

  • Resident physician means a physician enrolled in an internship, residency or fellowship.

  • Hospice services means palliative and supportive care and other services provided by an interdisciplinary team under the direction of an identifiable hospice administration to terminally ill hospice patients and their families to meet the physical, nutritional, emotional, social, spiritual, and special needs experienced during the final stages of illness, dying, and bereavement, as defined in Minnesota Statutes, § 144A.75, subd. 8, and includes the set of services as determined by the Medicare program under §1861(dd) of the Social Security Act and defined in 42 CFR § 418.3.

  • Community mental health program means all mental health

  • Bona fide physician-patient relationship means a treatment or counseling relationship between a physician and patient in which all of the following are present:

  • Behavioral therapy means interactive therapies derived from evidence-based research, including applied behavior analysis, which includes discrete trial training, pivotal response training, intensive intervention programs, and early intensive behavioral intervention.

  • Medical Specialist means any medical practitioner who is vocationally registered by the Medical Council under the Health Practitioners Competence Assurance Act 2003 in one of the approved branches of medicine and who is employed in either that branch of medicine or in a similar capacity with minimal oversight.

  • Diagnostic Service means an examination or procedure performed to obtain information regarding the medical condition of an outpatient.

  • Child in need of services means (i) a child whose behavior, conduct or condition presents or results

  • Human service zone means a county or consolidated group of counties administering human services within a designated area in accordance with an agreement or plan approved by the department.

  • Medical cannabis pharmacy means the same as that term is defined in Section 26-61a-102.
