FAP Application definition

FAP Application means the infor- mation and accompanying documenta- tion that an individual submits to apply for financial assistance under a hospital facility’s FAP. An individual is considered to have submitted a com- plete FAP application if he or she pro- vides information and documentation sufficient for the hospital facility to determine whether the individual is FAP-eligible and an incomplete FAP application if he or she provides some, but not sufficient, information and documentation to determine FAP-eli- gibility. The term ‘‘FAP application’’ does not refer only to written submis- sions, and a hospital facility may ob- tain information from an individual in writing or orally (or a combination of both).
FAP Application means theinformation and accompanying documentation that a hospital facility requires an individual to submit to apply for financial assistance under the facility’s FAP. A FAP application is considered complete if it contains information and documentation sufficient for the hospital facility to determine whether the applicant is FAP-eligible and incomplete if it does not contain such information and documentation.

Examples of FAP Application in a sentence

  • If Patient is granted 100% charity care without submitting a completed FAP Application and via presumptive scoring only, the amount of financial assistance for which Patient is eligible is limited to Patient’s unpaid balance after taking into account any payments made on Patient’s account.

  • A Patient may not be eligible for the financial assistance described in Paragraphs 1 through 3 above if such Patient is deemed to have sufficient assets to pay pursuant to an “Asset Test.” The Asset Test involves a substantive assessment of a Patient’s ability to pay based on the categories of assets measured in the FAP Application.

  • Make a reasonable effort to orally notify the Patient about the FAP and the FAP Application process.

  • Any pending ECAs shall be suspended during this time, and the written notice shall (i) describe the additional information and/or documentation required under the FAP or the FAP Application that is needed to complete the application, and (ii) include appropriate contact information.

  • Unless a complete FAP Application is submitted or eligibility is determined under the presumptive eligibility criteria of the FAP, the Organization will refrain from initiating ECAs for at least 120 days from the date the first post-discharge billing statement for the care is sent to the Patient.

  • In a situation where the Organization intends to defer or deny, or require a payment before providing, medically necessary care, as defined in the FAP, because of a Patient’s nonpayment of one or more bills for previously provided care covered under the FAP, the Patient will be provided a FAP Application and a written notice indicating that Financial Assistance is available for eligible Patients.

  • In the case of a Patient who submits an incomplete FAP Application, the Organization shall notify the Patient in writing about how to complete the FAP Application and give the Patient thirty (30) calendar days to do so.

  • Once a completed FAP Application is received on a Patient’s account, the Organization will evaluate the FAP Application to determine eligibility and notify the Patient in writing of the final determination within forty-five (45) calendar days.

  • In the case of a Patient who submits a complete FAP Application, the Organization shall, in a timely manner, suspend any ECAs to obtain payment for the care, make an eligibility determination, and provide written notification, as provided below.

  • Except as provided below, a Patient may submit a FAP Application at any time with respect to emergency and other medically necessary care received from the Organization.
