A copy of the most recent Functional Behavior Assessment, Positive Behavior Support Plan, ISP or Service Agreement, Nursing Service Plan, and Individualized Education Plan (if available).
If a Functional Behavior Assessment has not already taken place, one must be conducted.
The date may be extended up to an additional 90 calendar days with approval from the individual and the individual's case manager to allow additional time for the completion of a Functional Behavior Assessment and Positive Behavior Support Plan.
Functional Behavior Assessment (FBA): An assessment (required under certain conditions in the Individual’s with Disabilities Education Act) that assists in determining why a student exhibits inappropriate behavior and leads to the development of a Behavior Intervention Plan.
For a student who has an IEP or a Section 504 plan, such student’s IEP team or Section 504 team shall discuss the incident and consider the need to conduct a Functional Behavior Assessment (FBA), develop a Behavior Intervention Plan (BIP) or amend either if already in existence.
