Older Adults:Older Adults (age 60 and over) includes those who have serious mental illness who are unserved or seriously underserved, not eligible for Medi-Cal, and who have a reduction in personal or community functioning, specifically including older adults who are:• Homeless, or at risk of homelessness; and/or• At risk of institutionalization, nursing home care, hospitalization and emergency room services.
