HOSPITALIZATION INSURANCE. The Board shall assume the full cost of premiums for Blue Cross/Blue Shield Community Blue PPO with the following riders: (1) XVA2 (2) CBMHP (3) CBPCM for all employees and dependents. This program shall be on a three (3) year trial basis beginning with the 1998/1999 school year. Complementary coverage benefits will be mandatory for employees/ dependents who turn 65. At age 65, employees and/or dependents shall be required to enroll in Medicare in order to qualify for a plan equivalent to BC/65. Employees shall be reimbursed for Medicare providing the cost of the combination of BC/65 and Medicare does not exceed the amount previously contributed by the Xxx Xxxx Board for employees or dependents to their 65th birthday. Any member of the bargaining unit may elect to waive the above health insurance at any time and will receive eighty-four dollars ($84) per month in lieu of this benefit provided thirty-four percent (34%) or more of the VDAEOP membership elect to waive this benefit. This payment will be made in a lump sum (not to exceed $1,008 per contract year). Such payment will be made in December, before Christmas. If less than thirty-four percent (34%) of the membership waive health insurance, the stipend will revert to seventy-five dollars ($75) per month, not to exceed $900 per contract year.
