Individual Family Service Plan definition

Individual Family Service Plan or “IFSP” means a written statement of services and supports to be provided to a child and the child’s family for children less than the age of three years who are eligible for the Arizona Early Intervention Program (AzEIP) to enhance the capacity of families and care givers to support the child’s development and engagement and participation in everyday routines and activities.
