Case Manager. Case Manager - the primary service coordinator for families assessed to receive non-County services. The Contractor will supervise and train the Case Managers to work with children and families. Case Managers will perform the following activities, including but not limited to: - Complete all County-provided training related to DR, as determined by the County, such as the Community Worker curriculum, DR process, FAST, CARE, MDT and Mandated Reporter Training (MRT). - Participate in on-going training activities, as required by the County. - Conduct standardized Path One family assessments using pre-FAST. As indicated, develop service plans through the use of strength-based assessment tools that identify family strengths and needs. - Work jointly with SWs to conduct standardized Path Two family assessments using pre-FAST. SW will conduct assessment including safety, risk and protective capacity using CAT, and develop service plans as indicated and described above. - Provide services to a caseload of no more than 20 cases per Case Manager. - Engage families in services through a strength-based working relationship by meeting with families in their homes whenever possible. - Identify, establish, and link families to services and resources including on-site services as well as neighborhood-based services. - Provide family support through home visitation and informational services (i.e., home-teaching, motivational support, parent education, coaching, supportive problem solving, when appropriate, linkages to drug and alcohol treatment programs and domestic violence services, etc.). - Conduct periodic reassessments and service plan updates as indicated. - Consult with other Family Resource Center (FRC) staff when clinical expertise is needed in development of service plans. - Close cases as appropriate, conducting post-FAST at time of closure. - Assist in coordinating transportation to appointments, meetings, and classes. - Participate in regular MDTs, Family Self Sufficiency Team (FSST) meetings, individual supervision sessions and other meetings as appropriate. - Identify and invite additional parties to FSSTs. - Participate in community outreach activities. - Maintain up-to-date case records in CARE system. - Develop and maintain case files in CARE system containing assessment information, case plan, and record of contacts with clients. - Perform other duties as assigned.

Case Manager. An OlyCAP case manager will be on site five days per week for four or more hours per day, and consistent with a client’s capability, will provide assistance to services to help the client transition into permanent housing. Working with the case manager the client will be able to regain self-reliance to live independently in permanent housing. By being able to come and stay at the shelter, it allows the case manager access to the client to triage them, and to assess and prioritize their needs. Case managers are trained in best practices. Based on a client assessment, and using other OlyCAP funding resources, a case manager helps the client obtain needed services like bus passes, laundry vouchers, either directly, or by referral to an agency that has those services. Case managers work with willing clients on their housing needs through a housing plan, and together, the case manager and client will identify available housing and subsidies, which helps the client learn housing search skills.
Case Manager. An OlyCAP case manager works with the homeless person to help set goals and transition into permanent housing and self-sufficiency as soon as possible. OlyCAP will prioritize which homeless persons receive these services by coordinated entry, which evaluates each homeless person by a vulnerability assessment, and which scores the most vulnerable.

Case Manager. OlyCAP will provide full time case managers, who are trained in best practices to help identify needs for the client, stabilize them and help them transition into permanent housing. Many of the clients entering the Xxxxxx Street Cottages shelter are traumatized and need time to stabilize before the case manager can start working on the clients goals to help get into permanent housing. Case managers work with homeless persons to stabilize them and work with them to enable them to achieve success through setting and working towards their goals, and overcoming the challenges that lead them to being homeless. The case manager and the client work towards getting them into their own permanent housing and employment.
Case Manager. The parties agree that the employees designated as Case Managers and any other employees currently designated as salaried will continue to be classified as exempt status employees with the following considerations. The following provisions only of the collective bargaining agreement shall be modified for employees referenced in this letter:
1) The work week will not normally exceed five calendar days in a calendar week;
2) Bargaining unit seniority will be defined as the as the last date of hire by the Hospital as an RN;

Contractor will develop outcome-driven, individualized service plans as indicated in the Family Connections model. ▪ Engage families in services through a strength-based working relationship by meeting with families in their homes once a week for 1 – 1.5 hours. ▪ Provide family support and informational services (i.e., home-teaching, motivational support, parent education, coaching, supportive problem solving, when appropriate, linkages to drug and alcohol treatment programs and domestic violence services, etc.). ▪ Assist in coordinating transportation to appointments, meetings, and classes. Provide at least one ride for each family, as well as bus tokens and taxi vouchers as needed. ▪ Conduct reassessments after 90 days. Contractor will determine if case needs to continue for another 90 days. ▪ Update service plan. ▪ Close cases as appropriate, conducting post-FAF at time of closure. ▪ Develop and maintain case files in the secure, web-based ETO system containing assessment information, case plan, and record of contacts with clients. ▪ Track and monitor case activities through the life of a case and case closure. ▪ Maintain up-to-date case records in ETO system. Case Manager. The principal of each Attendance Center must ensure that at all times a case manager has been identified to serve at the Attendance Center. In addition to the duties common to a case manager, the case manager must serve as the Attendance Center’s Section 504 Plan coordinator, as well as the local school district representative (“Local School District Representative”) at IEP meetings whenever an ODLSS district representative (“District Representative”) has not been designated as the District Representative for those meetings. In order to serve as the Local School District Representative at such IEP meetings, the case manager must have a valid Professional Educator License (“PEL”) in one of the following areas: Special Education (PK-Age 21) endorsement or School Support Personnel endorsement with a supervisory endorsement and two (2) years’ experience in the field of special education is preferred. The case manager of each Attendance Center (or case manager representative if case manager is not available) must attend all CPS case manager meetings, in particular for IEP and Section 504 Plan training, on a monthly basis. The Board shall notify the Charter School of the available dates, locations, times and topics for the CPS case manager meetings through several communication channels. Case managers shall provide special education teachers and paraprofessionals with monthly training from such meetings, as well as indicate to the Board or its designee the method for providing such training.
