Linkage to Care definition

Linkage to Care means a programs effort to successfully link a person newly diagnosed with HIV to HIV related medical care on a timely basis and, at a minimum, consists of referral and offer of assistance with making an initial medical appointment and addressing barriers to keeping that appointment.
Linkage to Care means persons who are tested and receive a positive result for HIV (including preliminary positive results) will be linked to, and assisted in, scheduling an appointment with a provider of the person’s choosing. Successful linkage to care is measured as attendance to case management and/or a medical appointment with a prescribing provider.

Successful linkage is measured as attendance at an HIV related medical appointment with a prescribing provider. May include referrals to non-HIV related services.
