Longitudinal Family Physician Services definition

Longitudinal Family Physician Services means the types of services typically provided by a family physician who provides longitudinal, relationship-based, comprehensive, family medicine care in a community-based setting (including medically required services to beneficiaries), aligned with the attributes of a Patient Medical Home.
Longitudinal Family Physician Services or “LFP Services” means the types of services typically provided by a full services community family physician who provides longitudinal health care services (including medically required services to beneficiaries) and those services associated with a Patient Medical Home, including the services described in Section 12 [Included Services] but excluding those services described in Section 13 [Excluded Services].
