Plan Specifications definition

Plan Specifications means the section of this plan which shows, among other things, the Eligibility Requirements, Eligibility Waiting Period, Elimination Period, Amount of Benefits, Minimum Benefit, and Maximum Benefit Period.
Plan Specifications means the RHFP® Plan Specifications document attached hereto and incorporated herein by reference, by which the Employer/Sponsoring Entity establishes or by which an Adopting Employer/Sponsoring Entity adopts a welfare benefit plan pursuant to the terms of this Plan for the benefit of its Eligible Employees.
Plan Specifications means a description of CLIENT’s Plan related to Prescription Drug benefits and coverage limitations, including the framework of policies, interpretations, rules, practices and procedures applicable to such benefits, required by CLIENT and set forth in a mutually agreed upon document.

  • Compounded Drug Claims may be subject to quantity limits, dollar thresholds or Prior Authorization (PA) restrictions, or the exception process as defined in the applicable Benefit Plan or Plan Specifications.

  • Equipment and materials shall be installed in compliance with the manufacturer’s recommendations, except where a higher quality of workmanship is required by the Plan Specifications and these Standards.


More Definitions of Plan Specifications

Plan Specifications means in relation to each Member the details of the following matters determined in accordance with Clause 2.1:
Plan Specifications means the information made available by PBF to Participating Pharmacy describing, among other things, the applicable processes and Covered Services under a particular Sponsor’s health benefit plan and all related amendments, provided by PBF to Participating Pharmacy in accordance with this Agreement. This information may include the amounts to be reimbursed to Participating Pharmacy for Covered Services; excluded items; Cost Sharing Amounts; benefit maximums; coverage or payment limitations; and other items, all subject to the actual requirements of that Sponsor’s health benefit plan.
Plan Specifications. – means a description of CLIENT’s Plan related to Prescription Drug benefits and coverage limitations, including the framework of policies, interpretations, rules, practices and procedures applicable to such benefits, required by CLIENT and set forth in a mutually agreed upon document.

Related to Plan Specifications

  • Contract Specifications means the principal trading terms in CFD (for example Spread, Swaps, Lot Size, Initial Margin, Necessary Margin, Hedged Margin, the minimum level for placing Stop Loss, Take Profit and Limit Orders, financing charges, charges etc) for each type of CFD as determined by the Company from time to time. The Contract Specifications appear on the Website.

  • Service Specifications means the following documents, as applicable to the Services under Your order: (a) the Oracle Cloud Hosting and Delivery Policies, the Program Documentation, the Oracle service descriptions, and the Data Processing Agreement described in this Schedule C; (b) Oracle’s privacy policies; and (c) any other Oracle documents that are referenced in or incorporated into Your order. The following do not apply to any non-Cloud Oracle service offerings acquired in Your order, such as professional services: the Oracle Cloud Hosting and Delivery Policies, Program Documentation, and the Data Processing Agreement. The following do not apply to any Oracle Software: the Oracle Cloud Hosting and Delivery Policies, Oracle service descriptions, and the Data Processing Agreement.

  • Standard Specifications means the City of Madera Standard Specifications, including attached details and amendments thereto.

  • TENDER SPECIFICATIONS means the Scope of Work, Special Instructions / Conditions, Technical specifications/requirement , Appendices, Site Information and drawings pertaining to the work and any other relevant reference in the Tender Document for which the Bidder are required to submit their offer.

  • The Specification means the Specification annexed to or issued with G.T.C.C. and shall include the schedules & drawings attached thereto as well as all samples and pattern, if any.
