Customer Contact means the individual(s), whether a contract manager, category consultant or otherwise, who are to act for the Customer as the day to day point of contact for communications from the Contractor to the Customer in relation to the Contract, being the individual(s) identified in Section 2 of the Contract Schedule or otherwise notified to the Contractor from time to time;
Provider Personnel means all persons employed or engaged by the Provider together with the Provider’s servants, agents, consultants and Sub-Contractors (and all persons employed by any Sub-Contractor together with the Sub-Contractor’s servants, consultants, agents, Provider’s and Sub-Contractors) used in the performance of its obligations under this Agreement;
Provider contract means any contract between a provider and a carrier (or a carrier's network,
Provider network means an affiliated group of varied health care providers that is established to provide a continuum of health care services to individuals;
Designated Contact means, the designated contact for the Bank in the Jurisdiction, as set out in Annex I of Part A of these EB Services Terms and Conditions (as may be amended from time to time);
Primary Contact means any recreational or other water use in which there is prolonged and intimate human contact with the water, such as swimming and water skiing, involving considerable risk of ingesting water in quantities sufficient to pose a significant health hazard. Primary contact also means any use of surface waters of the state for cultural, religious or ceremonial purposes in which there is intimate human contact with the water, including but not limited to ingestion or immersion, that could pose a significant health hazard.
Direct contact means the contact of persons with high voltage live parts.
Technical Contact The Reporter designates the individual listed below as the contact person for technical or other implementation coordination issues under this Agreement. The contact person shall be the point of contact for the CMS for any technical questions that may arise during the term of this Agreement. If the Reporter changes its technical contact person, the Reporter shall notify the CMS in writing within thirty (30) working days of the transfer and provide the information listed below for the new contact person.
Authorized Contact is a representative authorized by Customer to request service changes using procedure outlined herein.
Service Provider Personnel means and refers to Service Provider employees or subcontractors hired and maintained to perform Services hereunder.
Primary Care Provider (PCP) means a health care professional who is contracted with BCBSAZ as a PCP and generally specializes in or focuses on the following practice areas: internal medicine, family practice, general practice, pediatrics or any other classification of provider approved as a PCP by BCBSAZ. Your benefit plan does not require you to have a PCP or to have a PCP authorize specialist referrals.
Network Provider means a provider of health care, or a group of providers of health care, which has entered into a written agreement with the issuer to provide benefits insured under a Medicare Select policy.
Provider is any individual or company that provides professional or technical services.
Contact means a communication, direct or indirect, between an athlete agent and a student athlete, to recruit or solicit the student athlete to enter into an agency contract.
Provider panel means those providers with which a carrier contracts to provide health care services
New Provider means any replacement service provider or providers engaged to provide the Services (or part thereof) or substantially similar services or the Authority itself where the Services or substantially similar services or part thereof continue to be provided by the Authority after partial termination, termination or expiry of this Contract;
Sexual contact means any touching of an erogenous zone of another, including without limitation the thigh, genitals, buttock, pubic region, or, if the person is a female, a breast, for the purpose of sexually arousing or gratifying either person.
Provider election officer means an election officer who enters into a contract or
Provider group means a medical group, independent
Internet Service Provider (ISP) means an Enhanced Service Provider (ESP) that provides Internet Services.
Non-Network Provider means any hospital, day care centre or other provider that is not part of the network.
Provider Number means an identifying number issued to each homecare worker who is enrolled as a provider through the Department.
ProviderOne or “P1” means the system commonly referred to as the Medicaid Management Information System (MMIS), and is the federally approved system used by the Washington Medicaid program to pay provider claims for goods and services authorized under the State Plan. The MMIS is certified by CMS and is the primary information system used by HCA to pay for health care.
Key Management Personnel means persons having authority and responsibility for planning, directing and controlling the activities of the Company, directly or indirectly, including any Director (whether executive or otherwise) of the Company;
Communications service provider means a service provider that provides broadband service.
Indirect contact means the contact of persons with exposed conductive parts.
