Site of Service Exhibit definition

Site of Service Exhibit means the exhibit attached hereto that lists the sites in which services may be provided to State Health Plan Members under this Agreement. In the event that you have an existing a Network Participation Agreement with Blue Cross and Blue Shield of North Carolina at the time that you sign this Agreement, we will use your existing Site of Service Exhibit as the Site of Service Exhibit for this Agreement.
