Wellness Coordinator definition

Wellness Coordinator means an individual who specializes in assisting Clients with access 6 to a myriad of health care needs, nutrition resources, and other community supports. This individual 7 will be responsible for documenting the services required, as well as communicating the needs of 8 Clients to the Family Team.
