Family Team definition

Family Team means a group formed to meet the needs of a FSP eligible Client through 9 members, and other support individual(s) the family agrees to include on the team.
Family Team means people identified by the child or family as collectively possessing the technical skills, knowledge of the family, authority, and access to the resources necessary to organize effective services to build on strengths and meet the needs of the child/youth or family.
Family Team means a team that is comprised of family members, friends, foster parents, legal custodians, community specialists, and other interested people identified by the family and Department who join together to empower, motivate, and strengthen a family, and collaboratively develop and support a plan to achieve child safety, child permanency, and child and family well-being.

  • Interdisciplinary team means a group of persons with varied professional backgrounds who meet with the member to develop a comprehensive service plan to address the member’s need for services.
