GENERAL TERMS AND CONDITIONS OF SALE. Applicability of the general terms and conditions: Every sale of goods or services by AGC nv nv, just as the agreement to accept work is subject to the present terms and conditions of contract. By concluding an agreement, the customer agrees to the exclusive application of AGC nv' terms and conditions of contract, to the exclusion of all other terms and conditions. Derogation from this is only possible on express written agreement. The customer accepts any amendments and commits to the applicable terms and conditions of contract with every order at the time of concluding or extending the agreement according to the terms and conditions published on AGC nv' website. Amendment or annulment of one or more clauses, in whole or in part, will not result in renunciation or annulment of the remaining terms and conditions. Establishment of the contract: Offers are made without obligations. The prices are only valid for a term of 15 days, unless otherwise agreed in writing. The agreement is only validated upon acceptance of the order by AGC nv.

  • Wat betaalt u zelf? ● Vanaf 18 jaar geldt eigen risico. ● Een van onderstaande medische indicaties of situaties geldt voor u: ○ U heeft diabetes mellitus type 1 en bent jonger dan 18 jaar. ○ U heeft slecht ingestelde diabetes type 1: ondanks standaard controle heeft u een blijvend hoog HbA1c (meer dan 8% of hoger dan 64 mmol/mol). ○ U heeft diabetes mellitus type 1 of 2 en bent zwanger. ○ U heeft preconceptionele diabetes type 1 of 2 en een zwangerschapswens. ○ U heeft diabetes mellitus type 1 en ernstige hypoglykemieën en/of u bent ongevoelig om hypoglykemieën waar te nemen (hypo-unawareness). ● De voorwaarden van het meest recente ZN formulier zijn het uitgangspunt. ● Als u kunt aantonen dat u geen geschikte telefoon heeft, is vergoeding van een reader mogelijk. ● U krijgt dit hulpmiddel in eigendom; u bent de eigenaar. ● Kinderarts. ● Internist. ● Endocrinoloog. ● Diabetoloog. ● Voor de eerste verstrekking beoordeelt de gecontracteerd zorgverlener of u voldoet aan de voorwaarden. Een akkoordverklaring van ons is dan niet nodig. ● U heeft altijd een akkoordverklaring van ons nodig als u naar een niet-gecontracteerde zorgverlener gaat. Als u een hulpmiddel wil laten leveren door een zorgverlener met wie wij geen contract hebben gesloten, moet u altijd eerst een akkoordverklaring bij ons aanvragen. Dit geldt dus ook voor reparatie, vervanging of vervolglevering van uw hulpmiddel door een niet-gecontracteerd zorgverlener. Kijk voor de akkoordverklaring in de bijlage Algemene Voorwaarden, onderdeel Akkoordverklaring. ● Op onze website kunt u zien bij welke zorgverlener u terecht kunt voor deze zorg. Kijk voor de algemene uitsluitingen in de bijlage Algemene Voorwaarden, onderdeel Algemene uitsluitingen. Hulpmiddelen in eigendom (begrippen) ● Insulinepomp met toebehoren. ● Vergoeding van 100 % voor insulinepomp met toebehoren. Wij hanteren verschillende tarieven.
