If a work-related disability, to provide a completed Treatment Memorandum (form available from the Health Centre) within one (1) week of date of accident or illness. • If a non-occupational illness or injury, to provide a completed Weekly Indemnity claim form within one (1) week of your date of disability. • A return to work must be arranged at least one day in advance with your Supervisor or the Department Superintendent. An employee who fails to provide such advance notification will not be entitled to reporting pay if work is not available in the employee's classification. • When returning to work, to provide to your Supervisor or the Health Centre, a doctor's note indicating that you are fit to return to work.
