Add Probands. Once this status has been approved, the institution's physician can register the Probands using a Proband ID and enroll them directly for the Test, depending on the offer and availability within the respective institution. The physician is responsible for obtaining the necessary consent from each Proband to share the personal data and must ensure that all Probands participating in the Test have consented to the testing of the Swab Sample. The physician is also responsible to inform the Probands about data processing and to obtain any consent with regard to data processing. The form for the written order including data protection notice and informed consent form must be downloaded, printed out, signed by the Proband and kept in the patient file by the physician. within the Corona Test Portal, the physician solely confirms that the Proband has commissioned Labor Xx. Xxxxx to perform the Test and has given any optional consent with regard to data processing. The physician is responsible to register the Proband in the institution's account correctly and to assign the sample ID to the corresponding Proband ID when the sample is collected or delivered. It is the sole responsibility of the physician to assign the Swab Sample to the correct Proband. Neither Labor Xx. Xxxxx nor CENTOGENE assume responsibility for any mix-up of the Probands caused by the physician.
