Ambulatory Site of Service Locations. Any deductible for outpatient ambulatory surgery center services will be waived or reimbursed for employees and their family members who live and receive these services more than 45 miles* from an Ambulatory Site of Service location. This section shall expire when Ambulatory Site of Service locations are established throughout the State so that no employee or family member must travel more than 45 miles, as the crow flies, from their town/city of residence for Ambulatory Site of Service locations.
