Annual Project Report Template. The purpose of this Annex 3 is to provide the Applicant PI with a template of the Annual Project Report Form that will need to be completed and submitted to UK Biobank on an annual basis (the annual anniversary of the Effective Date). For the avoidance of doubt, the Applicant PI is not required to complete this form on execution of the MTA.   • Progress Report 10.1 If required, you shall submit progress reports in connection with the Service (“Report”) on at least a monthly basis, or as we may require. The Report shall include a summary of the activities and accomplishments during the previous reporting period.
