Common use of Change of Residence Clause in Contracts

Change of Residence. Exhibit P CONTRACTOR shall notify parents in writing of their obligation to notify CONTRACTOR of changes in pupil’s residence. CONTRACTOR shall notify XXX in writing of pupil’s and/or parent’s change of residence, including any addition of a group home and license, within three (3) days after CONTRACTOR becomes aware of a pupil’s change in residency. CONTRACTOR shall also notice new Sonoma County district of residence within three (3) days by confirming pupil’s and/or parent’s DOR at http:/xxx.xxxxxx-xxxxxx.xxx/xxxxxxxx/xxxxxxxxxxxxxx.xxx. This Contract shall terminate as to any pupil upon the termination of the pupil’s residency within LEA’s boundaries. CONTRACTOR shall refer to Exhibit P for NPS duties and procedures for NPS students relocating to other districts. If the pupil’s new residence is located outside of LEA’s boundaries and CONTRACTOR fails to follow the procedures specified in this provision, XXX shall not be responsible for the costs of services delivered after the pupil’s change of residence if CONTRACTOR knew of the pupil’s change of residence. When the CONTRACTOR becomes aware of an emergency change of placement (e.g. hospitalization, juvenile hall, etc.), CONTRACTOR shall verbally notify LEA immediately (within 24 hours) and shall confirm the same in writing within three (3) days. When the CONTRACTOR becomes aware that a pupil placed into an LCI by another governmental agency and attending its NPS has changed residency, CONTRACTOR shall verbally notify LEA immediately (within 24 hours) and shall confirm the same in writing within (3) days on LEA approved forms.
