Common use of Check-Off Form Clause in Contracts

Check-Off Form. During the life of this Agreement and in accordance with the terms of the form of authorization of check-off of dues or service fee, as provided by the Union, subject to advance approval of the Board, the Board agrees to deduct Union membership dues or service fees levied in accordance with the constitution and by-laws of the Union from the pay of each employee who executes or has executed an authorization for check-off of dues or service fees form.
