Completion of Testing Form. a. The Employee may be asked by the collection facility to furnish only such information in writing as is necessary to ensure the integrity of the specimen collected, including information verifying the identity of the Employee and, if possible, identifying any prescription or nonprescription medication or alcohol recently ingested by the Employee.
