Contract Monitoring Division Forms. All proposals submitted must include the following Contract Monitoring Division (CMD) Forms contained in the CMD Attachment 2: Form 2A - CMD Contract Participation; Form 2B - CMD “Good Faith Outreach” Requirements Form; Form 3 - CMD Non-Discrimination Affidavit; Form 4 - CMD Joint Venture Form (if applicable;) and Form 5 - CMD Employment Form. Please submit Two (2) digital copies of these forms on two separatge CDs in .pdf format of the above forms with your proposal. The CD’s should be placed in a separate sealed envelope labeled “CMD/LBE Forms - CS-388 River Ecosystem Science Support” and delivered with the proposal package. One (1) original and one (1) copy of the CMD/12B form (Form No. 12B-101) must be submitted in a separate sealed envelope labeled “CMD/12B Forms - CS-388 River Ecosystem Science Support” and delivered with the proposal package. Failure to complete, sign and submit each of the CMD forms listed above may result in the proposal being deemed non-responsive and rejected.

  • Contract Monitoring The criminal background checks required by this rule shall be national in scope, and must be conducted at least once every three (3) years. Contractor shall make the criminal background checks required by Paragraph IV.G.1 available for inspection and copying by DRS personnel upon request of DRS.

  • Program Monitoring The Contractor will make all records and documents required under this Agreement as outlined here, in OEC Policies and NHECC Policies available to the SRO or its designee, the SR Fiscal Officer or their designee and the OEC.   • STATEWIDE CONTRACT MANAGEMENT SYSTEM If the maximum amount payable to Contractor under this Contract is $100,000 or greater, either on the Effective Date or at any time thereafter, this section shall apply. Contractor agrees to be governed by and comply with the provisions of §§00-000-000, 00-000-000, 00-000-000, and 00- 000-000, C.R.S. regarding the monitoring of vendor performance and the reporting of contract information in the State’s contract management system (“Contract Management System” or “CMS”).   • Implementation Report Within 150 days after the Effective Date, Extendicare shall submit a written report to OIG summarizing the status of its implementation of the requirements of this CIA (Implementation Report). The Implementation Report shall, at a minimum, include:

  • PERFORMANCE MANAGEMENT SYSTEM 5.1 The Employee agrees to participate in the performance management system that the Employer adopts or introduces for the Employer, management and municipal staff of the Employer.

  • Configuration Management The Contractor shall maintain a configuration management program, which shall provide for the administrative and functional systems necessary for configuration identification, control, status accounting and reporting, to ensure configuration identity with the UCEU and associated cables produced by the Contractor. The Contractor shall maintain a Contractor approved Configuration Management Plan that complies with ANSI/EIA-649 2011. Notwithstanding ANSI/EIA-649 2011, the Contractor’s configuration management program shall comply with the VLS Configuration Management Plans, TL130-AD-PLN-010-VLS, and shall comply with the following:

  • Account Monitoring Merchant acknowledges that Servicer will monitor Merchant’s daily deposit activity. Servicer may upon reasonable grounds suspend disbursement of Merchant's funds for any reasonable period of time required to investigate suspicious or unusual deposit activity. Servicer will make good faith efforts to notify Merchant promptly following suspension. Servicer is not liable to Merchant for any loss, either direct or indirect, which Merchant may attribute to any suspension of funds disbursement.

  • Project Monitoring The Developer shall provide regular status reports to the NYISO in accordance with the monitoring requirements set forth in the Development Schedule, the Public Policy Transmission Planning Process Manual and Attachment Y of the OATT.
