Coverage under Medicaid or CHIP for an Eligible Employee and/or Eligible Dependent Sample Clauses

Coverage under Medicaid or CHIP for an Eligible Employee and/or Eligible Dependent is terminated as a result of loss of eligibility for such coverage, and coverage is requested under this Contract no later than sixty (60) days after the date of termination of such coverage.
