Deficiencies Identified Through Formal Observations. Observations resulting in identification of performance deficiencies shall be followed within five (5) days by a conference between the evaluator and the member in order for questions arising from the observation to be discussed. All of the evaluator’s observations shall be compiled in writing. A copy of the written observation report shall be given to the member at the post-observation conference. The second and subsequent observation(s) of each evaluation where deficiencies that may lead to adverse personnel action have been identified will be conducted in the same or similar class as where the deficiency(ies) were initially observed. The evaluator and/or immediate supervisor involved in the particular area of the member’s work shall assist the member in attempting to correct the deficiencies. The evaluator shall submit a written plan for correcting the deficiencies which shall include ways in which the evaluator/supervisor shall assist the member to correct the deficiencies. The plan shall include a reasonable time between observations to allow time for improvement in the areas of performance deficiency (see OTES Model Improvement Plan). It is the responsibility of the supervisor to define the deficiencies and to provide a specific plan. It is the responsibility of the member to implement said plan.
