Common use of Documentary evidence for Sick Leave Clause in Contracts

Documentary evidence for Sick Leave. and Xxxxx’s Leave is required after the first single day’s absence in any year in relation to a period of Sick Leave or Carer’s Leave taken (or to be taken). (a) For Sick Leave the employee must provide the Company with satisfactory evidence such as a medical certificate confirming that he or she was, is or will be unable to attend work due to the employee being unfit for work because of personal illness or injury. (b) For Xxxxx’s Leave the employee must provide the Company with satisfactory evidence such as a medical certificate confirming that an Immediate Family Member had, has or will have a personal illness or injury during the period; or (i) if the care or support is due to an unexpected emergency affecting the Immediate Family Member – a Statutory declaration made by the employee will be required.
