For Sick Leave the Employee must provide the Company with satisfactory evidence such as a medical certificate confirming that he or she was, is or will be unable to attend work due to the Employee being unfit for work because of personal illness or injury.
For Sick Leave. Employees requesting sick leave must call the on-duty supervisor as soon as possible, but not less than one (1) hour prior to the start of the scheduled workday, and upon return to work, complete such forms as may be required by the Village. An employee may be required in the exercise of the Village's discretion to substantiate proof of illness when there is reason to suspect sick leave abuse. If Village of Wilmette & Teamsters Local 700 - Police Union Contract (2016-2018) an employee is absent for three (3) or more consecutive days or if the employee has a history of frequent absence, the employee may be required by the Village to bring in a doctor's note and require the employee to provide a prognosis from the doctor as to the earliest date when the employee will be able to return to work and perform the essential duties of the position or any restricted duty assignment. The Village may require an employee to have the doctor's statement updated should the employee not return to work within the time frame originally established by the physician. Any cost associated with providing a physician's statement is the responsibility of the employee. In the event of elective surgery which may result in the employee being away from work for a period of one week or more, the employee is required to give at least two week's notification to the Chief of Police. Time taken as sick leave as well as absences beyond an employee's accumulated sick leave, may qualify for treatment under the Family & Medical Leave Act. An employee may use up to twenty-four (24) hours of sick leave annually for the illness of a "family member," as defined in Section 10.4(a).
For Sick Leave. A doctor's certificate that the employee was unable to attend for duty on the day or days for which leave is claimed. Alternatively, a statutory declaration may be provided to substantiate three sick leave days each year (a year being 12 months from the anniversary date of the Agreement).
For Sick Leave. To be credited with a day and one half of sick leave for any month, an employee must have been at work for a total of not less than seventy-five hours. For purposes of this Article, an employee who is on a paid vacation during any month will be considered as been on duty. of Sick Leave Xxxx leave will be granted to a regular full time employee on the following basis: [a] Sick leave will not be available during an employee's probationary period. Commencing at the end of an employee's probationary period, one and one half 1/21 days sick leave for each month of active service will be granted retroactive to the first of the month following the date he was employed. The unused portion of sick in any year of service be accumulated up to a maximum of one hundred days. Cumulative sick leave may only be used for illness or non accident and will not be paid in advance. In order to qualify for leave, an employee must notify his Supervisor or the switchboard operator at least hour prior to the beginning of the employee's working day on the day shift and at least two hours prior to the beginning of the employee's working day on the afternoon or shift, and the Hospital reserves the right to require proof of illness from a qualified medical practitioner. [e] Employees will not be compensated for the first two days of illness on the third and succeeding periods of absence from work on sick leave in any one calendar year unless they provide proof of illness from a qualified medical On retirement, under provisions of the Pension Plan, the Hospital will pay one hundred percent of the value of the employee's accumulated sick leave 'days up to a maximum of ninety days.
