Common use of Drug Care Plan Clause in Contracts

Drug Care Plan. (a) The Employer and the Union agree to the continuation of the Drug Care Plan as follows: (i) Eligibility requirements for employees and dependents will be the same as the Dental Service Plan; (ii) Co-insurance will be based on eighty percent (80%) reimbursement; (iii) The maximum payment per contract (family) is six-hundred fifty dollars ($650). (b) Other terms and conditions of the Drug Care Plan will be similar to those formerly in effect for the drug coverage provisions of the employee-paid Extended Health Benefits plan.

(a) The Employer and the Union agree to the continuation of the Drug Care Plan effective October 1, 2001 as follows: (i) Eligibility requirements requirement for employees and dependents will be the same as those in effect for the Dental Service Plan; (ii) Co-insurance will be based on eighty percent (80%) reimbursement; (iii) The maximum payment per contract (family) is six-seven hundred fifty dollars ($650)700) effective January 1, 2015. (b) Other terms and conditions of the Drug Care Plan will be similar to those formerly in effect for the drug Drug coverage provisions of the employee-paid Extended Health Benefits plan(EHB) Plan.
