Common use of Extended Health Care Coverage Clause in Contracts

Extended Health Care Coverage. (A) The Employer shall pay one hundred percent (100%) of the monthly premiums for extended health care coverage for regular employees and their eligible dependents (including common-law spouses) under the Pacific Blue Cross Plan, or any other plan mutually acceptable to the Union and the Employer (See also Appendix “X”). The plan benefits shall be expanded to include: (1) expenses incurred for the purchase and maintenance of a hearing aid up to a maximum of six hundred dollars ($600) per person in each four (4) year period; and (2) Vision care coverage providing two hundred and twenty-five dollars ($225) every twenty-four (24) months per eligible employee or eligible dependent. (3) The maximum lifetime amount payable per eligible employee or eligible dependent shall be unlimited. (B) A regular employee may cover persons other than dependents if the plan carrier agrees and if the employee pays the full premium for them through payroll deductions. (C) Membership in the extended health care plan is a condition of employment for regular employees who are not members or dependents of members of another approved extended health care plan. (D) The extended health care plan becomes effective on the first of the calendar month following thirty (30) days from the date of hire.
