Common use of Extended Health Insurance Clause in Contracts

Extended Health Insurance. The Festival will pay 100% toward the cost of: Benefits Drug: Pay Direct Deductible per family unit $5.00 per prescription Reimbursement 100% Vision: $300 none 100% Hospital: xxxx to semi-private none 100% Supp. Health Care none 100% Out-of-Province Emergency none 100% • Maximum eligible expenses for eyeglasses/contact lenses every 24 month period (12 month period for each insured dependent under age 18). Eye exams to be covered through the insurance company; eyeglasses/contacts reimbursement through the Stratford Festival • Hearing aids shall be covered for both ears at the eligible expense of $500.00 each, every three (3) years • Other maximums are listed under the appropriate Provision page. Termination of Insurance - 65th birthday.
