Eyeglasses. (A) If, in the line of duty, eyeglasses are damaged or destroyed, replacement or repair will be in accordance with the following schedule: (1) If glasses are totally destroyed, the City will pay for replacement of lenses based upon the last verifiable prescription plus $50.00 for frames. (2) If either or both lenses are lost or destroyed, replacement will be made based on the last verifiable prescription. (3) In the event of damage, loss or destruction of frames, the City will pay a maximum of $50.00 for replacement or repair. (B) All claims for payment or reimbursement will be submitted to the designated officer on the proper form.
