Family Planning Services and Supplies. The Contractor shall provide family planning services and supplies in accordance with AMPM Policy 420, and consistent with the terms of the 1115 Waiver, for all members of reproductive age regardless of gender who choose to delay or prevent pregnancy. These include medical, surgical, pharmacological and laboratory services, as well as contraceptive devices. Information and counseling, which allow members to make informed decisions regarding family planning methods, are also included. [42 CFR 457.1230(d), 42 CFR 438.210(a)(4)(ii)(C)]. If the Contractor does not provide family planning services and supplies due to moral and religious objections, it shall Contract for these services through another health care delivery system or have an approved alternative in place, or AHCCCS will disenroll members who are seeking these services from the Contractor and assign them to another Contractor. Members may choose to obtain Family Planning Services and Supplies from any appropriate provider regardless of whether the Family Planning Service Providers are network providers. The Contractor shall not require PA to allow members to obtain family planning services and supplies from an out-of-network provider. The Contractor shall submit a Sterilization Report as specified in AMPM Policy 420 and Section F, Attachment F3, Contractor Chart of Deliverables.
