Final Evaluation Procedures. 1. Each teacher’s performance rating will be combined with the assessment of student growth measures to produce the summative evaluation rating, based upon the “Evaluation Matrix” adopted by the Ohio Department of Education. 2. The evaluator shall provide that each evaluation is submitted to the teacher for his/her acknowledgment by written receipt. If signed by the teacher, the receipt is to be sent to the Superintendent as soon as received.
